The recipient, a 46-year-old male with diabetes, hypertension, and chronic kidney disease, was undergoing maintenance dialysis and faced challenges in finding a compatible ‘O’ blood group donor within his family. After exploring other options, the medical team proceeded with an ABO-incompatible renal transplant, with the donor being the patient’s 38-year-old wife of ‘B’ positive blood group.


Speaking on the success of this breakthrough procedure, Dr. P. Arun Prasath, Consultant Nephrologist at Apollo Speciality Hospitals, Madurai, said, “ABO-incompatible kidney transplantation is a highly complex procedure that requires precise planning and execution. By adopting advanced antibody removal protocols and immunosuppressive therapy, we were able to overcome the immunological barrier and achieve excellent outcomes for the patient.”


To prepare for the transplant, the recipient underwent a specialized protocol involving immune-suppressing medications and plasmapheresis. The transplant surgery, performed on December 18, 2024, was a resounding success, with the patient exhibiting good urine output immediately post-surgery and achieving normal renal function by the fourth postoperative day.
